Unofficial transcripts from ALL academic institutions attended, undergraduate and graduate.

Please note: CAPS is unable to accept transcripts that have been copied/pasted into other programs (such as Microsoft Word).

A copy of your Financial Aid Award (only if you have been awarded work study).

An academic writing sample (only if you are applying to be a writing tutor).

Resume/CV

A recommendation from a professor, instructor, or faculty member in the subject area for which you want to tutor/SI. Your recommendation may be from a Teaching Assistant (TA) if and only if they are listed as the instructor of record for the course in the course catalog.

*If any required documents are missing from an application or are deemed unacceptable, CAPS holds the right to either request new documents or remove the application from consideration. If new documents are requested, an email will be sent to the applicant and will result in a delay of the application being reviewed.
