Instructor Notification Form

The Instructor Notification Form grants permission for the Center for Teaching and Learning to release information about student visits to course instructors or program coordinators. The INF is often used to show proof of visits to our Center as part of course or program requirements. Students must grant permission for this information to be released as part of the Family Educational Rights and Privacy Act (FERPA), which is a law that protects the privacy of student education records.

If you are a student, you only need to complete this form once per semester per recipient (your instructor or program coordinator). If you have any questions about the INF, please contact Moumita Ghosh.

Instructor Notifications are sent out three times per semester; the Spring dates are:

  • March 1, 2024 at 5pm 
  • April 5, 2024 at 5pm
  • May 3, 2024 at 12pm

Please do not wait to use our services if visits are required for your class. Visits occurring on the last day of tutoring services may not be included on your report, which are prepared and sent to instructors before the end of the day.

For Faculty and Instructors: If you are an instructor who is requiring visits as part of your course, we recommend that you contact Moumita Ghosh to set up a consenting process and custom scheduled report for your entire class.
